Updated Service Agreements

Business Banking Services Agreement are now available for your records. In order to ensure your Agreement is accurate, please complete the below information with the appropriate individuals by May 15, 2022.

Our agreements require two separate individuals sign on behalf of the entity. The two signers are referred to as the Authorized Representative and Certifying Person. 

Unless otherwise requested, documents will be sent for electronic signature via DocuSign with the subject line: Please DocuSign: Simmons Bank Treasury Management Agreement(s). Signers will receive an authentication code at the cell phone numbers entered in the form below.

Authorized Representative

The Authorized Representative should be the person authorized in the entity documentation to sign legally binding agreements on behalf of the entity (often a president, member, manager, or partner). The Authorized Representative will be the person to sign all agreements moving forward.

Certifying Person

The Certifying Person is an individual that can certify that the Authorized Representative has the authority to sign on behalf of the entity (often a secretary, partner, or member).
